  • Identity & Contact — Full name, date of birth, phone number, email address, and home address. Used to identify you and communicate about your services.
  • NDIS Details — Participant number, NDIS plan dates, funded support categories, and budget amounts. Used to manage and claim your NDIS funding accurately.
  • Financial Information — Bank or payment details for plan management billing and NDIA claims. Used to process invoices and submit claims on your behalf.
  • Health & Disability — Health conditions, disability-related needs, medications, and clinical requirements relevant to your supports. Used to deliver safe, appropriate care.
  • Support Network — Emergency contacts, guardian or nominee details, and significant others involved in your care. Used to coordinate your support and respond in emergencies.
  • Goals & Preferences — Support goals, service preferences, communication needs, and any cultural or religious considerations. Used to tailor your care plan to your individual circumstances.

We collect this information directly from you, your authorised representative, or other NDIS service providers involved in your care — always with your consent.

2How We Use Your Information

We use your personal information to:

  • Deliver NDIS plan management and support coordination services
  • Process and submit funding claims to the NDIA on your behalf
  • Communicate with you about your services and appointments
  • Comply with NDIS registration and reporting obligations
  • Manage our internal business operations and quality assurance
  • Respond to complaints and feedback

We do not use your information for direct marketing without your explicit consent.

3Disclosure of Your Information

We may share your information with:

  • The National Disability Insurance Agency (NDIA) as required for plan management
  • Other service providers involved in delivering your supports, with your consent
  • The NDIS Quality and Safeguards Commission for regulatory compliance

We take reasonable steps to protect your information from misuse, interference, loss, and unauthorised access. Staff with access to your information are required to maintain confidentiality.

In the event of a data breach that is likely to result in serious harm, we will notify the Office of the Australian Information Commissioner (OAIC) and affected individuals in line with the Notifiable Data Breaches scheme.
